

Substance Use Disorder Assessment
A substance use assessment is for those who are interested in exploring how substance use impacts their relationships, mental health, and daily functioning. A thorough evaluation is conducted by a licensed drug and alcohol counselor focused on providing treatment recommendations. The assessor will give you personalized recommendations that match the determined level of care needed, which may include mental health services, harm reduction strategies, and/or community resources.
What to Expect:
Prior to being scheduled, you will be given questionnaires to complete through the online portal. A one-hour interview will be scheduled and conducted to gather information. Additional questionnaires may be given to gather additional information either from the client and/or others in their life to get another perspective. Records that may be requested for the assessment include medical records (including current medication information), relevant legal records, and biographical records.
